Susan Hutson has been indicted on 30 felony counts connected to a May jailbreak in New Orleans in which 10 inmates escaped.
Prosecutors allege her failures and lack of proper precautions contributed to conditions that allowed the escape, even though she did not directly open any jail doors.
Her co-defendant, CFO Bianka Brown, was also indicted on related charges tied to alleged false records and obstruction of justice.
The escape involved inmates breaking out through defective cell locks and a wall opening, and all were eventually recaptured, NBC News has reported.
Hutson and Brown are scheduled for court proceedings as officials continue reviewing systemic security failures at the jail.
